New online triage appointment system

From Wednesday 23 August 2023 we will be introducing a new online triage appointment system.

We will transition from a ‘first come first serve’ model to a ‘total triage’ model. This means appointments will be booked in based on clinical need rather than who gets through on the phone first.

Instead of calling or coming to the practice, you will be able to send in a consultation online via e-consult which can be accessed on our website or on the NHS App.

If you do not wish to use our online system, you are still able to call the practice for an appointment however please note that the reception team will not book an appointment but will complete an online consultation on your behalf.

All incoming online consultations will then be triaged by the doctor who will decide on the most appropriate mode of care delivery.

We aim to respond to all consultations within 48 hours and if the doctor feels it is necessary to contact you as a priority, we will be in touch sooner. If you have submitted an online consultation, there is no need to call the practice to check we’ve received your request, please be assured we will be contact you.

Thank you for your patience and understanding during this time. These changes are being made to improve our access and to help our patients to gain the most appropriate advice and assistance.

Registering for online services – Add links to forms if required

To register for online services, so you can book appointments, order medication and view your medical record, fill in the online access application form. You can also download and complete the form and bring it into the practice.

The practice will phone or text you to confirm a GP has authorised you to view your medical record.

You will then need to bring photo ID and proof of your address to the surgery, where the receptionist will print your online access codes.

If you are a newly registered patient, we will have to wait for your medical record to arrive at the surgery before we can process your request to view your medical record.

Registering for someone else on their behalf – Add links to forms if required

You can register for someone else on their behalf (proxy access) if you are a carer or relative.

To register on behalf of someone else, complete the online proxy access request form. You can also download the form and bring it into the practice.

Proxy access needs to be authorised by a GP. The practice will phone or text you to confirm the request has been authorised.

You will then need to bring photo ID and proof of your address to the surgery, where the receptionist will set up your proxy access.
